Official DTAM reporting formsPT28 records controlled cannabis flower used at or supplied from the establishment. Each row needs the transaction date, recipient details, purpose, relevant licence information, and dried-flower quantity.
Understand the PT27 / PT28 record trailPT33 is a medical cannabis prescription issued by an authorized practitioner after a medical assessment. It is separate from PT27/PT28 reporting and cannot be generated here. Cannabox MD offers medical review; prescription approval is not guaranteed.
