Who uses it and when

The licence holder prepares this report from use or outgoing transactions. Check recipient details and supporting evidence, including prescriptions when relevant, before monthly reporting. Confirm deadlines and the accepted submission channel with DTAM or your provincial public health office.

What each field means

1. Reporting month and year

   Enter the month covered by the transactions and the Buddhist Era year.

2. Receiving authority

   Choose the central or provincial registrar for your submission and enter the province when relevant.

3. Licence holder

   The name and licence number in the header belong to the reporting business, rather than the buyer.

4. Transaction date

   Use the date flower was used or supplied, supported by the original transaction record.

5. Buyer or recipient

   Check the name, Thai ID or passport number and date of birth against the form instructions. Legal entities do not need a date of birth.

6. Purpose

   Mark the actual purpose, such as personal use, resale, research, processing or export. Selecting a box does not replace authorization evidence.

7. Buyer's licence

   Enter it when applicable. The printed instructions exempt certain cases such as personal use; check that your transaction qualifies.

8. Flower quantity

   Use the actual outgoing quantity in dried grams. Check it against the sale or use record and prescription when relevant.
